FDC Assault Advisories | May 16 - 22, 2017

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty.

MONTICELLO, Fla. – On May 16, an inmate assaulted a correctional officer at Jefferson Correctional Institution.

Inmate Jarrod Slick (Q31490) assaulted the officer at approximately 9:17 p.m. ET. Inmate Slick head-butted the officer in the face. Staff responded appropriately, and the inmate was subdued.

Medical staff examined the officer and noted a cut on the nose. Inmate Slick will receive a disciplinary report for this assault.


PUNTA GORDA, Fla. – On May 16, an inmate assaulted a correctional officer at Charlotte Correctional Institution.

Inmate Rudy Martinez (Y53311) assaulted the officer at approximately 7:01 a.m. ET. Inmate Martinez elbowed the officer in the chest. Staff responded appropriately, and the inmate was subdued.

Inmate Martinez will receive a disciplinary report for this assault.


LIVE OAK, Fla. – On May 16, an inmate assaulted a correctional officer at Suwannee Correctional Institution.

Inmate Johntaviuos Taylor (V34477) assaulted the officer at approximately 4:32 p.m. ET. Inmate Taylor punched the officer in the face with a closed fist. Staff responded appropriately, and the inmate was subdued.

Inmate Taylor will receive a disciplinary report for this assault.


LAKE CITY, Fla. – On May 17, an inmate assaulted a correctional officer at Columbia Annex.

Inmate Jose Cruzado (L61619) assaulted the officer at approximately 5:15 p.m. ET. Inmate Cruzado became combative and bit the officer on the hand. Staff responded appropriately, and the inmate was subdued.

Medical staff examined the officer and noted minor injuries. Inmate Cruzado will receive a disciplinary report for this assault.


PERRY, Fla. – On May 18, an inmate assaulted a correctional officer at Taylor Correctional Institution.

Inmate Raheim Brockington (N25159) assaulted the officer at approximately 4:43 p.m. ET. Inmate Brockington head-butted the officer in the chest. Staff responded appropriately, and the inmate was subdued.

Inmate Brockington will receive a disciplinary report for this assault.


MALONE, Fla. – On May 19, an inmate assaulted a correctional officer at Jackson Correctional Institution.

Inmate Kithanie Fulwiley (H05995) assaulted the officer at approximately 1:35 p.m. CT. Inmate Fulwiley became aggressive and struck the officer in the head multiple times. Staff responded appropriately, and the inmate was subdued.

Medical staff examined the officer and noted swelling and scratches on the head. Inmate Fulwiley will receive a disciplinary report for this assault.


OKEECHOBEE, Fla. – On May 20, an inmate assaulted a correctional officer at Okeechobee Work Camp.

Inmate Johnathan Jones (W38389) assaulted the officer at approximately 2:55 p.m. ET. Inmate Jones refused to submit to restraints and attacked the officer, taking him to the ground in the process. Staff responded appropriately, and the inmate was subdued.

Medical staff examined the officer and noted an injured rib. Inmate Jones will receive a disciplinary report for this assault.


WEWAHITCKA, Fla. – On May 20, an inmate assaulted a correctional officer at Gulf Annex.

Inmate Carl Polaske (L39716) assaulted the officer at approximately 2:55 p.m. ET. Inmate Polaske charged the officer and began striking him repeatedly. Staff responded appropriately, and the inmate was subdued.

Medical staff examined the officer and noted minor injuries. Inmate Polaske will receive a disciplinary report for this assault.


MILTON, Fla. – On May 21, an inmate assaulted correctional officers at Santa Rosa Annex.

Inmate Dominique Kelsey (J51597) assaulted the officers at approximately 7:25 a.m. CT. Inmate Kelsey struck one officer in the face with a closed fist and kicked another officer in the abdomen. While being restrained, the inmate brandished an improvised weapon and cut one of the officers on the hand. Staff responded appropriately, and the inmate was subdued.

Medical staff examined the officers and noted injuries. Inmate Kelsey will receive a disciplinary report for this assault.


LAKE BUTLER, Fla. – On May 21, an inmate assaulted a correctional officer at Reception and Medical Center West Unit.

Inmate Michael Moore (L89120) assaulted the officer at approximately 12:45 a.m. ET. Inmate Moore elbowed the officer in the forehead. Staff responded appropriately, and the inmate was subdued.

Inmate Moore will receive a disciplinary report for this assault.


CHIPLEY, Fla. – On May 21, an inmate assaulted correctional officers at Northwest Florida Reception Center.

Inmate Steven Burns (124614) assaulted the officers at approximately 9:51 a.m. ET. Inmate Burns kicked the officers in the legs. Staff responded appropriately, and the inmate was subdued.

Inmate Burns will receive a disciplinary report for this assault.


CHIPLEY, Fla. – On May 22, an inmate assaulted correctional officers at Northwest Florida Reception Center.

Inmate Brandolen Miller (N21574) assaulted the officers at approximately 5:58 a.m. ET. Inmate Miller and four unknown inmates attacked the officer and kicked the officer. Staff responded appropriately, and the inmate was subdued.

Medical staff examined the officer and noted injuries. Inmate Miller will receive a disciplinary report for this assault.
