FLORIDA DEPARTMENT OF CORRECTIONS

FDC Assault Advisories | August 3 – August 09, 2022

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty. All assaults are investigated, and disciplinary action is imposed in accordance with policy.

Zephyrhills Correctional Institution: On August 3, Inmate Fernandito Carpio (X81705) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Charlotte Correctional Institution: On August 3, Inmate Johnathon Peavy (K86510) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Zephyrhills Correctional Institution: On August 3, Inmate Fernandito Carpio (X81705) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Central Florida Reception Center: On August 3, Inmate Joseph Jackson (T46053) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Reception and Medical Center: On August 4, Inmate Joshua Hutchins (I10604) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Zephyrhills Correctional Institution: On August 4, Inmate Sergio Castro (V36577) assaulted an officer by kneeing them in the knee. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Charlotte Correctional Institution: On August 4, Inmate Marcus Bryant (P24972) assaulted officers by kicking them in the arm. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Wakulla Annex: On August 5, Inmate Willie Nelson (M74641) assaulted a staff member. Staff responded appropriately, and the inmate was subdued.

Union Work Camp: On August 6, Inmate Jonathan Carver (U11307) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Mayo Correctional Institution: On August 6, Inmate Tyrese Freeman (C90002) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Polk Correctional Institution: On August 6, Inmate Lazaro Hernandez (M80990) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Gulf Correctional Institution: On August 7, Inmate Torey Anderson (040488) assaulted an officer by striking them in the eye. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Santa Rosa Annex: On August 7, Inmate Mario Santiago (E61409) assaulted a staff member. Staff responded appropriately, and the inmate was subdued.

Putnam Correctional Institution: On August 7, Inmate Andrew Hampton (D21913) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Cross City Correctional Institution: On August 8, Inmate Shelton Hanna (456319) assaulted an officer by striking them on the head. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Reception and Medical Center: On August 8, Inmate Larry Muldrow (789775) assaulted a staff member. Staff responded appropriately, and the inmate was subdued.

Apalachee Correctional Institution: On August 8, Inmate Justin Brinson (134261) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

South Florida Reception Center: On August 9, Inmate David Clark (F00029) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Charlotte Correctional Institution: On August 9, Inmate Andre Gomez (U29864) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Suwannee  Annex: On August 9, Inmate Michael Eady (135095) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Desoto Annex: On August 9, Inmate Lorenzo George (128672) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Putnam Correctional Institution: On August 9, Inmate Tommy Blash (424251) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Liberty Correctional Institution: On August 9, Inmate Devonte Louis (L84481) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
