FLORIDA DEPARTMENT OF CORRECTIONS

FDC Assault Advisories | May 31 – June 6, 2022

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty. All assaults are investigated, and disciplinary action is imposed in accordance with policy.

Jackson Correctional Institution: On May 31, Inmate Bruce Williams (U48770) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Florida State Prison: On May 31, Inmate Tremaine Bailem (Y08089) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Florida Women’s Reception Center: On May 31, Inmate Shateryka Washington (N32349) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Florida State Prison: On June 1, Inmate James Alexander (A51787) assaulted an officer by biting them. Staff responded appropriately, and the inmate was subdued.

Florida State Prison: On June 1, Inmate Antwon Anderson (C05253) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Polk Correctional Institution: On June 1, Inmate Jazmaro Mason (R88591) assaulted an officer by pushing them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Santa Rosa Correctional Institution: On June 2, Inmate Stepfon Smith (N21103) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Okeechobee Correctional Institution:On June 2, Inmate Ernest Coleman (308353) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued.

Dade Correctional Institution: On June 3, Inmate James Broomfield (Y00623) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Jackson Correctional Institution: On June 3, Inmate Demetrius Morrison (B15645) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Blackwater River Correctional Facility: On June 3, Inmate Yoan Toscano (B07508) assaulted an employee. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Annex: On June 3, Inmate Gerald Davis (121130) assaulted an officer by elbowing them in the cheek. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Gulf Correctional Institution: On June 5, Inmate Andre Edwards (C06452) assaulted an officer by kicking them in the leg. Staff responded appropriately, and the inmate was subdued.

Northwest Florida Reception Center: On June 6, Inmate Robert Grantham (R78455) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Walton Correctional Institution: On June 6, Inmate David Douglas (X89479) assaulted officers by throwing urine on them and striking them in the ear with his fist. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officers and noted injuries.

Martin Correctional Institution: On June 6, Inmate Tex Foster (Y60764) assaulted an officer by striking them in the torso with his shoulder. Staff responded appropriately, and the inmate was subdued.
