FDC Assault Advisories | August 24 – August 30, 2021

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty. All assaults are investigated, and disciplinary action is imposed in accordance with policy.

Florida State Prison: On August 24, Inmate Freddy Velazquez (193038) assaulted two officers by striking them. Staff responded appropriately, and the inmate was subdued.

Okeechobee Correctional Institution: On August 24, Inmate Sirron Jackson (J60430) assaulted an officer by attacking them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Santa Rosa Annex: On August 24, Inmate Diego Martinez (M61093) assaulted two officers by attacking them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Taylor Annex: On August 25, Inmate Chaz Dunlap (T64175) assaulted an officer by striking them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Tomoka Correctional Institution: On August 27, Inmate Michael McKenzie (128903) assaulted an officer by headbutting them. Staff responded appropriately, and the inmate was subdued.

Charlotte Correctional Institution: On August 29, Inmate Jorge Navarrete (B13974) assaulted an officer by headbutting them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.
