FLORIDA DEPARTMENT OF CORRECTIONS

FDC Assault Advisories | April 17 - April 23, 2020

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty. All assaults are investigated, and disciplinary action is imposed in accordance with policy.

Lake Correctional Institution: On April 17, Inmate William Quinn (R52609) assaulted an officer by striking them in the leg with a food tray. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Correctional Institution: On April 17, Inmate Keith Perkins (R84640) assaulted an officer by headbutting them. Staff responded appropriately, and the inmate was subdued.

Sumter Correctional Institution: On April 19, Inmate Ernesto Hernandez (D70613) assaulted an officer by striking them in the face with a closed fist. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Annex: On April 21, Inmate Orlan Hagins (K57435) assaulted an officer by striking them in the head. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Dade Correctional Institution: On April 21, Inmate Lazarus Calhoun (815269) assaulted an officer by striking them in the face with a closed fist. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Florida State Prison:  On April 21, Inmate Jonathan Burton (Y50899) assaulted an officer by headbutting them and kicking them in the shin. Staff responded appropriately, and the inmate was subdued.

Walton Correctional Institution: On April 21, Inmate Jeffrey Williams (458928) assaulted an officer by striking them in the face.  Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Santa Rosa Annex: On April 21, Inmate Juan Valerio (D57556) assaulted an officer by striking them in the stomach. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Correctional Institution:  April 22, Inmate Rafael Quintanilla (135531) assaulted two officers by striking them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officers and noted injuries.

Santa Rosa Work Camp:  On April 23, Inmate Anthony Goods (Q60673) assaulted an officer by kicking the . Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Desoto Annex:  On April 23, Inmate Harold Sears (U58020) assaulted an officer by striking their hand. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Central Florida Reception Center :  On April 23, Inmate Raymond Frolander (V47736) assaulted an officer by striking them with a closed fist. Staff responded appropriately, and the inmate was subdued.
