FDC HIPAA Notice - Glades Correctional Institution
TALLAHASSEE, Fla. - On March 16, 2017, x-ray reports and films of 80 inmates housed at Glades Correctional Institution from 1990-2008 were discovered to have been donated by an unknown person or persons to a third party retailer for resell.┬ب These reports and films contained images of the body and information about symptoms and/or diagnoses
On March 27, 2017, the Department was made aware of this matter through a formal communication, and conducted a thorough investigation in accordance with Federal requirements. After completing this investigation, the following corrective actions were taken: the non-redacted copy of these records were secured, the inactive records were appropriately destroyed, HIPAA compliance for staff was recommended and information was retained in order to report this event as part of the Department’s Annual HIPAA Report to the U.S. Department of Health and Human Services.
It has been determined that the records which contain protected health information are no longer unsecured.
Notification via U.S. certified mail was sent to those individuals affected by the recent breach.┬ب This substitute notice follows.
